Documents Required for the Active Duty Military Application

These are required documents for this property. Please review the list as you are preparing to submit your application.

STEP 1: Complete the FHO Application

Contact the Family Housing Office (FHO) to complete a government housing application. You may reach them by phone at (808) 496-2676 or email at MCBH.G4.FMLY.HSG.FMB@usmc.mil.

You can complete the application in person at the FHO, or you may submit an application online through email. Visit the following website for more information: https://www.mcbhawaii.marines.mil/MCBH-Living/#tab/family-housing.

Documents needed for the FHO application:

  • Copy of PCS orders.

You will need to complete the following forms, which are available on the FHO website:

  • DD1746: Application for Assignment to Housing
  • Registered Sex Offender Disclosure Statement
  • MCBH Family Housing Applicant's Statement of Understanding (SOU)
  • Privacy Act Release Letter

Using your personal email address on the application is strongly recommended to avoid communication issues with your .mil account.

  • If completing the application on behalf of the active-duty service member, a valid Power of Attorney (POA) is required.
  • If a spouse, children, or dependents are not listed on orders, a marriage certificate, birth certificate, or proof of pregnancy must be provided.

STEP 2: Begin the HMC Application

After your FHO application is approved, you will receive a welcome email from Hunt to begin your HMC application. If you are a Priority 1-Priority 2 applicant, you will receive the link within 24 hours of applying with the FHO and having your application approved. If you are a Priority 3-Priority 7 applicant, you will need to check in with the Ohana Marines Family Housing Office before receiving the link for the HMC application.

THE APPLICATION PORTAL MAY NOT BE SUPPORTED ON GOVERNMENT COMPUTERS OR MOBILE DEVICES.

STEP 3: Gather the Documents Needed & Submit Your HMC Application

  • All applicants must provide proof of Assignment Date with Official Orders.
  • All applicants must present a Current Leave Earning Statement (LES) to confirm current rank.
  • If all dependents (spouse/kids) are not listed on orders, you must provide the following documents:
    • Marriage certificate, DD1172, or 10922
    • Birth certificates for every person under 18
    • *If all dependents are listed on your orders, no additional documents are needed.
  • If you have pets, you must provide all of the following for each pet:
    • Shot records
    • Photo
    • *If your pet is a service animal or emotional support animal, supporting documentation is required.
  • If you are filing the information on behalf of your active-duty spouse, you need a power of attorney (POA).
  • If you are dual-military, you must provide orders and a current LES for both members. One family member must be on another's orders; if not, you will need to provide a marriage certificate or DD1172.

STEP 4: Waitlist Placement and Move-In Requirements

Once Hunt approves your application, you will be placed on a waitlist for homes that you qualify for until you are offered a home. You will receive confirmation once you are placed on the waitlist.

All Priority 1 & 2 service members are eligible for a pre-lease home offer up to 30 days prior to arrival on the island. The service member must check in with Family Housing before keys can be issued.
